Update your contact details
Keeping your address and phone number up to date helps us reach you with appointments, test results and important health information.
Please let us know as soon as possible if you change your address, phone number or email.
You can update your details by visiting reception, calling us, or emailing the practice. If you use the NHS App, you can also check and update some details online.
How to tell us
Visit reception with proof of your new address if you have moved. If you prefer, call or email us with your full name, date of birth and the details that have changed.
