Common questions about registering with a GP in Hayes

Am I in your practice area?

You usually need to live in our practice area. The online form checks your address first, so the quickest way to find out is to start it. If you live just outside, call us on 0203 794 3119 and we'll see what we can do.

Do I need ID or an NHS number?

No. You don't need ID, proof of address or immigration documents, and you don't need to know your NHS number. If you use the paper form, a copy of your passport or birth certificate helps us process it faster. You can also find your NHS number online.

How long does registration take?

About 15 minutes for the form, then we'll confirm within a few days. You can book as soon as we do.

Does it cost anything?

No, it's completely free to register.

What about my regular medication?

Keep at least a month's supply when you switch. Your records can take around a month to reach us, and you might not be able to renew prescriptions during that time.

I'm only in Hayes for a short while. Can I still see a GP?

Yes, if you're here for less than 3 months you can ask to join as a temporary patient. Call us on 0203 794 3119 to arrange it.

Do I need to tell my current GP?

No. We'll let your old practice know for you, and your records move across automatically.

Can I register my children too?

Yes. Parents and guardians can register their children, and each adult fills in their own form.
